Milan compare to other italian cities is very european, so english is quite well used in main places (banks, --restaurants, museums, train ticket office). Pubblic offices can be very bad in english and you can have hard time without italian: you should refer to your ambassy for these documents (taxes, VISA). For the every day life it really depends in your job and wirking environment.
